Fantastic book, reads like a techno-thriller. Well researched and absolutely compelling, this book tells two stories. One is the blow-by-blow account of a Titan II missile accident in Damascus, Arkansas, that involved a 9 megaton thermonuclear warhead and is edge-of-the-seat exciting. The other is the most complete history of nuclear warhead safety that's ever been printed. I had considered myself well-informed, but this book really opened my eyes. My highest recommendation.
